Formación

• Master's in Nursing Practice or equivalent qualification.
• Current SANC registration as a Nurse Practitioner.

• Proficient in using electronic health record systems.

About the Role

Our client, a leading community-focused healthcare provider serving the Mossel Bay, Western Cape area, is seeking a highly skilled and compassionate Community Health Nurse Practitioner . This vital role is dedicated to delivering comprehensive primary healthcare services, health education, and preventative care to individuals and families within the community. Key Responsibilities

• Conduct comprehensive health assessments, including medical history, physical examinations, and diagnostic tests.
• Diagnose and treat acute and chronic illnesses within the scope of practice for Nurse Practitioners.
• Develop and implement evidence-based treatment plans, prescribing medications as appropriate.
• Provide counseling and education to patients and families on health promotion, disease prevention, and management of chronic conditions.
• Perform routine health screenings, immunizations, and wellness checks.
• Refer patients to specialists or other healthcare facilities when necessary.
• Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
• Collaborate effectively with physicians, specialists, community health workers, and other members of the healthcare team.
• Participate in community outreach programs and health fairs to promote health awareness and access to care.
• Advocate for patient needs and ensure continuity of care.
• Stay current with advancements in healthcare, evidence-based practices, and relevant regulations.
• Contribute to the development and implementation of community health initiatives in Mossel Bay.

Requirements

• Master's degree in Nursing Practice or equivalent qualification from an accredited institution.
• Current registration with the South African Nursing Council (SANC) as a Nurse Practitioner.
• Minimum of 5 years of experience as a Nurse Practitioner in a primary care or community health setting.
• Demonstrated expertise in diagnosing and managing a wide range of health conditions across the lifespan.
• Strong understanding of public health principles and community-based healthcare delivery.
• Excellent clinical assessment, diagnostic reasoning, and treatment planning skills.
• Proficiency in electronic health record (EHR) systems.
• Exceptional communication, interpersonal, and patient education skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of the healthcare landscape and community resources in Mossel Bay, Western Cape .
