Job summary

Madrona Hospice Care Inc. is seeking an LPN/LVN to provide per diem home hospice care. You will deliver direct patient care at patients' homes, following individualized plans of care and educating families on self-care.

The ideal candidate is a graduate of an accredited practical nursing program with an active CA LPN/LVN license, 3+ years of nursing experience (home health or med/surg preferred), and CPR certification. Must have a valid driver’s license and be willing to travel frequently.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle; willingness to travel to patient homes.
• Strong communication and collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
