Job summary

D.I.V.E Opportunity Homes is seeking an Intensive Care Coordinator (ICC) to lead cross-system, trauma‑informed care coordination for youth in treatment. You will work with a multi‑agency team to plan, implement, and track services, ensuring engagement with families and community partners.

The role requires a Bachelor’s or Master’s in Social Work or a related field, strong organizational skills, and willingness to travel as needed.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or other mental health discipline required.
• Experience navigating child/family-serving systems with state agencies or behavioral health facilities.
• Experience advocating for children/families with serious emotional/behavioral health needs.

Responsibilities

• Coordinate cross-system care with youth treatment teams across agencies.
• Facilitate Child and Family Team Meetings (CFTs) and planning activities.
• Provide Targeted Case Management and aftercare support; link to resources and referrals.
