The New Jewish Home seeks a full-time Workforce Programs Case Coordinator for its SkillSpring department in New York. You will engage with young adult and high school participants, assess needs, and help plan services to support education and employment goals.

The role emphasizes compassionate care coordination, collaboration with internal teams and external partners, and ongoing assessment to adapt plans as needs change. Some morning, evening, or weekend hours may apply.

Responsabilités

• Ensure the health, safety and well-being of young adult participants by supervising activities.
• Screen and identify participants in need of care coordination and conduct comprehensive assessments.
• Collaborate with SkillSpring teams to identify problems requiring planning, intervention or follow-up.
• Support participant needs and promote effective use of resources; facilitate involvement in care.
• Identify and arrange appropriate services with knowledge of local and federal resources.
• Coordinate care plan implementation with care conferences and community partners.
• Monitor participant needs and revise care plans as conditions change.
• Document assessments, plans and responses; evaluate outcomes with the team.
• Participate in process improvement, benchmarking and education material development.
• Maintain and improve care management skills and work with SkillSpring staff.
• Support youth programming including professional development and health career exploration.
• Monitor attendance and survey data for programming; assist with other activities.
• Create a diverse, culturally sensitive environment for young adults.
• Perform other related duties as required.
