Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field is required.
• One year of experience working with individuals experiencing mental illness and/or substance use is preferred.
• Passion for mental health advocacy and recovery-oriented care is essential.

Responsibilities

• Build trusting relationships with individuals and meet them where they are.
• Use a trauma-informed approach to coordinate supports.
• Develop, implement, and update individualized recovery plans.
• Liaise with therapists, healthcare professionals, and case managers for coordinated care.
• Monitor progress toward goals and provide psychosocial support.
• Maintain regular contact via in-person visits, calls, and follow-ups.
• Assist with transportation access for program appointments.
• Document interactions accurately in line with standards.
• Complete required county and agency trainings.
