Green Chimneys is seeking a Transition Coordinator at the Brewster Main Campus to provide comprehensive casework and coordination for youth transitioning from residential treatment services to family and community supports.

You will work with RTC youth and guardians, arranging community-based services and ensuring smooth handoffs from intake to reentry into the community. Position is regular full-time with benefits.

Qualifikationen

• Bachelor’s Degree in Human Services.
• 2 years’ experience providing direct services to or linking children with serious emotional disturbance to services.

Aufgaben

• Provide comprehensive casework services for transitioning RTC youth and families/guardians.
• Coordinate with community providers to establish linkages and supports from intake through community reentry.
