Job summary

The SKIL Resource Center in Parsons is seeking a Youth Transition Coordinator to provide and coordinate Independent Living Services for youths transitioning to independent living. You will work with the center, schools, families and agencies to empower youth to set goals and access community resources.

You will develop Individual Living Plans, teach life and job-readiness skills, and advocate for youths while coordinating services at home, school and in the community.

Qualifications

• Must be at least 18 years old.
• Have a valid Driver's License.
• Be familiar with Independent Living philosophy.
• Clear background checks.

Responsibilities

• Provide the five core services: Individual and Systems Advocacy, Independent Living Skills Training, Peer Counseling, Information and Referral and Deinstitutionalization.
• Assist youth with community resources such as housing, transportation, jobs and other resources as needed.
• Develop and update Independent Living Plans with youth, focusing on strengths, interests and individual goal setting.
• Provide assistance in identifying and coordinating existing services to achieve Independent Living goals.
• Teach independent living skills and job readiness skills, such as effective communication, teamwork and problem-solving.
• Identify barriers to community integration and pursue removal through the Agency's ADA Advocacy Implementation Plan.
• Work with youth's support network team when developing person-centered plans and goals.
• Empower youth to identify needs, gain problem-solving skills and advocate for themselves.
• Act as liaison between schools, families, employers and agencies involved in helping the youth.
