Job summary

Skil-Resource-Center in Parsons, KS seeks a Youth Transition Coordinator to provide and coordinate Independent Living Services for youth aging out of high school or facilities, helping them live independently and reach self-directed goals within their chosen community.

You will work with schools, families and agencies, develop and update Individual Living Plans, connect youth to housing, transportation and job resources, and teach IL and job-readiness skills while advocating for youth’s needs.

Qualifications

• Must be at least 18 years old.
• Have a valid Driver's License.
• Be familiar with Independent Living philosophy.
• Clear background checks.

Responsibilities

• Provide the five core services: Individual and Systems Advocacy, Independent Living Skills Training, Peer Counseling, Information and Referral and Deinstitutionalization.
• Assist youth with community resources such as: Housing, Transportation, Jobs and other resources as needed.
• Develop and update Independent Living Plans with youth, focusing on strengths, interests and individual goal setting.
• Provide assistance in identifying and coordinating existing services that will assist in achieving the independent living (IL) goals established in the Independent Living Plan.
• Teach independent living (IL) skills and job readiness skills, such as effective communication, teamwork and problem‑solving.
• Identify barriers to integration within the community and actively pursue the removal of those barriers through the Agency’s ADA Advocacy Implementation Plan.
• Work with youth’s support network team as needed when assisting youth in developing a person-centered plan and achieving desired goals.
• Empower youth to identify their own needs, gain problem‑solving skills and advocate for themselves.
• Work as a liaison between schools, families, employers and agencies involved in helping the youth.

Summary

Provide and coordinate Independent Living Services to youth transitioning from high school, a facility or an institution to live independently within the community of their choice and empower those individuals to reach self-directed goals. Assist youth in individual and group settings, both at the Center and in the individuals’ home and community with person-centered plans, connect youth to community resources, and build self-advocacy skills. Work closely with schools, families, and agencies who are involved in helping the youth with decision-making, goals and developing independent living skills.
