Job summary

Vinfen is seeking a Transition to Independence Facilitator to support transitional age youth and young adults through the TIP program. You will provide coaching, person-centered planning, and advocacy to help YYAs meet life goals.

You will collaborate with schools, educators, and mental health professionals to coordinate services, develop individualized plans, and promote successful outcomes for YYA as they transition into independence.

Qualifications

• Minimum of 2 years’ experience in educational, social service, juvenile justice, and/or mental health settings with counseling, training, therapy, and/or guidance to YYA and families (Bachelor’s degree) or minimum of 1 year experience (Master’s degree).
• One year working in a strengths-based model serving YYA as Facilitator.

Responsibilities

• Conducts strength-based needs assessments via Strength Discovery Assessment for transition domains.
• Coaches and supports 12-15 YYA through TIP levels.
• Delivers evidence-informed practices as guided by program leadership.
• Collaborates with programs and educators to support YYA activities.
• Records data on YYA progress across goals and statuses.
• Assists YYA in developing TIP Core Competency Plan.
• Participates in IEP planning and advocacy when YYA is in high school.
• Develops prevention plans for at-risk YYA.
• Interagency collaboration to ensure services for YYA.
• Brokers necessary services per TIP Core Competency Plan.
• Promotes parent involvement in planning services.
• Interfaces with adult mental health case managers for age-appropriate services.
• Identifies obstacles to independent living, employment, and education and develops strategies.
• Manages program resources and works flexible hours; participates in on-call duty.
