Job summary

Vinfen is seeking a dedicated facilitator to conduct strength-based needs assessments for YYA, develop individualized TIP Core Competency Plans, and coordinate supports across education, mental health, and social services. The role emphasizes advocacy, collaboration with multidisciplinary teams, and ensuring smooth transition planning for youth and young adults.

Qualifications

• Minimum 2 years' experience in educational, social service, juvenile justice, and/or mental health settings providing counseling, training, therapy, or guidance to YYA and families (with a Bachelor's degree) or minimum 1 year with a Master's.
• One year working in a strengths-based model serving YYA as Facilitator; experience with YYA with emotional/behavioral dysregulation or psychiatric illness encouraged.

Responsibilities

• Conducts strength-based needs assessments and develops individualized TIP Core Competency Plans.
• Facilitates person-centered planning and supports transition planning for YYA.
• Collaborates with school personnel, teachers, and counselors to develop advocacy and planning for YYA during school years.
• Coordinates services and supports with public and private agencies to ensure access to community resources.
• Maintains transition records and charts contacts per Vinfen standards.
• Promotes parent involvement and engages key players in planning services for YYA.
• Interfaces with adult mental health case managers to ensure age-appropriate services as YYA approaches 18.
