Job summary

Vinfen Corporation in Massachusetts is hiring a Facilitator for the Transition to Independence (TIP) program. You will provide mentoring, skills coaching, contingency contracting, counseling, and problem solving to transitional age youth and young adults, helping them build person-centered plans and achievable goals.

You’ll work with YYA, families, teachers, and service partners to coordinate supports, track progress, and adjust plans as needed.

Qualifications

• Bachelor's or Master's degree with relevant experience.
• Minimum 2 years in educational, social service, or mental health settings.
• Experience with Strengths-based model serving YYA and their families.
• Driving is a requirement for this position.

Responsibilities

• Conducts strength-based needs assessments via the Strength Discovery Assessment process.
• Coaches and supports 12-15 YYA through TIP levels: active, maintenance and follow-along.
• Delivers evidence-informed practices as directed by program leadership.
• Collaborates with other service practitioners and educators to ensure appropriate services.
• Charts and records all contacts and services per Vinfen standards.
• Promotes parent involvement and engagement of key players in planning.
