Job summary

Casa Pacifica Centers For Children & Family in Camarillo, CA is seeking a Transition Facilitator to implement the clinical case management component of the treatment plan for transitional-age youth.

Under the Director of Transitional Youth Services, you will coordinate TILP milestones, engage youth with strengths-based approaches, connect families to resources, and document progress while upholding Casa Pacifica’s standards.

Qualifications

• Bachelor's degree required; experience in social services preferred.
• Experience with transitional-age youth and case management preferred.
• Bilingual in Spanish preferred.

Responsibilities

• Partner with social workers and probation officers to advance TILP development for transitional-age youth.
• Engage youth to build rapport through a strength-based approach and create individualized TILP.
• Advise youth on personal rights and self-advocacy; foster independence and life skills.
• Attend TILP transition conferences and address individual needs within Extended Foster Care.
• Provide in-the-moment guidance on parenting and connect families to resources (childcare, insurance, etc.).
• Maintain documentation of services and meet care minute goals; participate in trainings.
