Job summary

Casa Pacifica is seeking a Transition Facilitator to implement the clinical case management component of the treatment plan for transitional- age youth. The role involves coordinating with social workers and probation officers to develop individualized TILP and assisting youth in advocating for themselves.

A California driver’s license is required, and bilingual Spanish is preferred. The position emphasizes collaboration with community partners and documentation of services, while upholding Casa

Qualifications

• A valid California driver’s license is required to transport clients.
• Bachelor’s or equivalent in a related field with significant experience in human services; BSW/MSW preferred.
• Certified Social Work Case Manager (C-SWCM) or Certified Advanced Social Work Case Manager (C-ASWCM) is a plus.
• Proficiency with computers and Microsoft Office Suite.

Responsibilities

• Collaborate with social workers and probation officers to advance transitional age youth plans.
• Engage youth and emerging adults to build rapport and develop individualized TILP.
• Assist youth in advocating for themselves and understanding personal rights.
• Attend transition conferences and address individual goals within Extended Foster Care.
• Provide in-the-moment guidance for parenting and connect families with community resources.
