Job summary

Turning-Point-Community-Programs in California seeks a direct service professional to assist members with crisis resolution and wellness in the community. This at-will position provides support in medication management, transportation, counseling and advocacy as needed.

Responsibilities include coordinating care for Transition Aged Youth, completing assessments and developing treatment plans, delivering individual and group therapy, and ensuring documentation meets funder standards.

Qualifications

• Master's degree in Social Work, Psychology, or related field.
• Two years of experience as a provider of mental health services.
• California driver’s license and current vehicle insurance/registration.
• Reliable transportation capable of passing vehicle safety inspection if driving.
• Schedule: Mon–Fri, 8:30 am - 5:00 pm with rotating on-call responsibilities.
• Travel as needed for client transport and program support.

Responsibilities

• Assess, coordinate, and facilitate supports for Transition Aged Youth and families to decrease crises.
• Completes required paperwork and assessments.
• Develops and participates in interdisciplinary team meetings.
• Completes tasks related to fidelity to the START Model and program certification.
• Coordinates care with system partners and outside agencies.
• Develops individual treatment plans and provides individual/group therapy.
• Meets clients at Turning Point sites and other locations; transports clients as needed.
• Provide on-the-spot counseling and crisis intervention when required.
