Job summary

BCFS Health and Human Services seeks a Lead Transition Specialist to oversee transition services for youth aged 16–21, applying best practices and program standards in crisis situations.

You will develop Individual Service Plans, monitor progress, and ensure compliance with DFPS PAL guidelines while mentoring staff and coordinating with the Director of Community Based Services. Strong communication and organizational skills are required.

Qualifications

• Bachelor's degree in social services or related field.
• 2+ years of experience in case management, job development or crises intervention with youth.
• Experience in community building, event coordination and public speaking.

Responsibilities

• Provide case management to young adults 16–21 eligible for the program.
• Develop detailed Individual Service Plans and track progress.
• Obtain approval for DFPS youth participation before services.
• Follow DFPS guidelines and PAL policies.
• Mentor direct reporting staff to promote performance.
• Ensure staff training for program quality.
• Attend meetings and present on transition services.
