Job summary

Wayside’s TEMPO - Framingham seeks a dynamic Case Manager/Transitional Facilitator to provide a variety of case management and related services to homeless young adults (18-24) in transitional housing. You will coordinate intake assessments, hold weekly case management meetings, and develop tailored service plans with participants.

The role requires 1+ year experience in residential settings or programs for homeless youth; driving is required; bilingual abilities are highly preferred.

Qualifications

• Bachelor's degree preferred, not required.
• Bi-lingual capacity extremely preferred.
• Must have at least 1 year experience with youth/adult populations.
• Experience with residential programs for homeless individuals is required.

Responsibilities

• Coordinate intake assessments for new participants.
• Schedule weekly case management meetings with participants.
• Develop, implement, and evaluate service plans with participants.
• Provide life skills assessment and education/training for participants.
• Provide employment assistance and apprenticeship placement.
• Build and maintain a network of community-based support services.
• Provide counseling, crisis intervention, referrals, and advocacy.
• Monitor participants and provide follow-up services up to six months.
• Complete all required documentation and attend agency meetings.
• Assist with policy development and ensure policy compliance.
