Job summary

Qualifications

Responsibilities

• Provide in-home case management, advocacy, safety planning, and housing/education/employment planning for Young Adults.
• Coordinate services with the treatment team to ensure comprehensive care.
• Link youths to community resources and assist with housing, landlord coordination, and progress monitoring.
• Teach essential independent living skills like transportation and budgeting.
