Job summary

The State of Indiana's Department of Child Services seeks an Older Youth Case Manager to support adolescents' transition to adulthood across Allen County and Northeast Indiana. You will assess progress, maintain case documentation, plan for needs and build support systems, including potential court hearings.

Qualifications

• Associate degree with 2+ years of human services work experience, or
• 30 college credits with 4+ years of human services experience, or
• 5+ years of human services experience.

Responsibilities

• Investigate reported child abuse/neglect and determine substantiation.
• Assess risk to a child and determine need for protective action.
• Perform needs assessments to determine treatment options.
• Testify and prepare reports for court or case management teams.
• Develop case plans to help families achieve self-sufficiency.
• Monitor services and coordinate with providers and partners.
• Accept on-call duties to respond to allegations as needed.
• Visit children in out-of-home placements and coordinate reunification or adoption.
