Job summary

Portage County is seeking a full-time CST Service Coordinator to wrap around youth with complex needs within the child welfare system. You will regularly meet with youths and caregivers, assess strengths and needs, and implement a person-centered care plan with a Coordinated Service Team.

The role requires a bachelor’s in social work or related field, Wisconsin social work certification eligibility, and driving access. Competitive benefits are provided by Portage County.

Qualifications

• Bachelor's degree in social work or related field required.
• Wisconsin social worker certification or eligibility preferred.
• Experience in child protective services preferred.
• Knowledge of child development preferred.
• Experience in mental health preferred.
• Must have access to insured vehicle.

Responsibilities

• Meet with youth and caregivers regularly and provide ongoing wraparound support.
• Coordinate and facilitate Coordinated Service Team with measurable goals.
• Refer families to appropriate services within the agency or community.
• Consult with therapists, school staff, and other providers to monitor progress.
• Complete case and permanency plans as requested and document in the system.
• Maintain required assessments and data in the reporting system.

Skills

Valid driver's license

Education

Bachelor's degree in social work or related human services field

Certified social worker or eligible for certification in Wisconsin

Experience in child protective services

Knowledge of child development
