Job summary

Old Colony YMCA is seeking a Caseworker (Full Time) for a 17-21 year old group home in Fall River, Massachusetts. The role focuses on supervision, safety, and helping youths develop daily living skills while coordinating medical and educational appointments.

The candidate will liaise with families, DCF, schools, and medical providers to meet each youth’s needs, document thoroughly, and participate in weekly reviews.

Qualifications

• Bachelor’s degree candidate in a Human Services related field or with 1–5 years of applicable experience.
• Strong knowledge of Microsoft programs is required.
• Excellent reading and writing skills for contract and incident reporting.
• Ability to communicate with clients, families, and collaterals in person and by phone.
• Daily attendance and scheduling flexibility.
• Must be able to drive and have a valid driver’s license.

Responsibilities

• Advocate for clients in home, school, and community according to treatment plan.
• Identify community providers and medical/dental needs after intake.
• Schedule medical/dental appointments as needed.
• Coordinate medication orders and immunization documentation.
• Facilitate medical/dental/eye appointments to maintain health.
