Wisconsin Counties is seeking a qualified professional to plan, provide, arrange, and monitor services for consumers in CCS, CST, and CLTS programs, under supervision. The role emphasizes trauma-informed and person-centered care, with a focus on improving outcomes and coordinating care across providers.

Responsibilities include assessments, treatment planning, crisis management, and family outreach, with travel within the county to support community-based services.

Competenze

• Bachelor’s degree in behavioral science and 1,000 hours of supervised post-degree clinical experience.
• Bachelor’s degree in another field with 2,000 hours of supervised post-degree clinical experience.
• Knowledge of trauma-informed principles preferred.
• Knowledge of federal laws, state statutes, and administrative rules governing services.
• Ability to coordinate activities across units and agencies.
• Valid driver’s license and reliable vehicle; travel required within county.

• Perform clinical assessments including intake and yearly reviews.
• Develop and implement treatment plans and crises using trauma-informed approaches.
• Ensure culturally and linguistically appropriate service delivery.
• Provide information and outreach to youth and families.
• Refer consumers to contracted services and monitor progress toward independence.
• Document assessments and treatment plans accurately and timely.
