Rock Creek Foundation seeks a Youth Personal Supports Counselor to provide one-on-one support for youth and transitional-age youth in Plum Orchard, Silver Spring, MD. You will work across homes, schools, and community settings under Person-Centered Plans.

The role emphasizes trauma-informed, culturally responsive, and family-inclusive care, requiring travel and flexible scheduling to meet program needs.

Qualifications

• High School Diploma or equivalent.
• Must complete Maryland Board of Nursing CMT course.
• Must pass DDA core trainings.
• Must have valid driver’s license & auto insurance.
• Frequent local travel required.

Responsibilities

• Provide one-on-one youth support ages 13+ in various settings.
• Deliver HIPAA-compliant virtual support to improve functioning.
• Provide in-person overnight support when needed.
• Help youth build social, daily living, and self-advocacy skills.
• Assist youth at medical/behavioral health appointments.
• Participate in Person-Centered Planning and goal setting.
• Coordinate with families, schools, and providers.
