Job summary

The Rock Creek Foundation in Silver Spring, MD is seeking a dedicated youth support specialist to provide individualized one-on-one support for youths 13+ with developmental disabilities or behavioral challenges. You will deliver HIPAA-compliant virtual support and may provide in-person overnight care as needed.

You will help youths build social, daily living, and personal care skills, coordinate with families and professionals, and document services per DDA guidelines.

Qualifications

• High School Diploma (or equivalent) and/or one (1) year of related experience.
• Knowledge of the Internet and Microsoft Office software required
• Must be able to be trained on Electronic Medical Record software
• Must be able to successfully pass the Maryland Board of Nursing CMT course, as well as other DDA core trainings.
• Must possess a valid driver’s license and proof of current automobile insurance
• Frequent local travel is required as part of the day-to-day responsibilities of this role

Responsibilities

• Provide individualized one-on-one support to youth ages 13 and older with developmental disabilities, emotional and behavioral challenges, or co-occurring needs.
• Deliver HIPAA-compliant virtual support to maintain or improve functional abilities, strengthen social connections, and promote participation in activities.
• Provide in-person overnight support where required.
• Support youth in building skills across core development areas including social and communication skills, activities of daily living and personal care.
• Assist youth in attending medical, behavioral health, and psychiatric appointments.
• Actively participate in Person-Centered Planning processes and contribute to goal setting.
• Work with youth, caregivers, schools, providers, and community partners to reinforce skill development.
• Maintain accurate documentation of all services per DDA requirements and policies
• Ensure services align with the youth’s Person-Centered Plan and program limitations
