Job summary

Pathways to Housing DC is seeking an Outreach Specialist to provide field-based engagement with youth aged 18–24 experiencing homelessness in Washington, DC. You will connect youth to housing and essential resources, supporting Housing First and trauma-informed care.

The role requires 75% fieldwork, late-night shifts, and strong collaboration with community partners to maintain access to housing, benefits, and services. Equal opportunity employer.

Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, Sociology, or a related field.
• Strong interpersonal, documentation, and computer skills.
• Excellent written and verbal communication skills with the ability to effectively engage clients, community partners, and multidisciplinary teams.
• Demonstrated cultural competence and the ability to provide respectful, person-centered, and trauma-informed services to individuals from diverse backgrounds and lived experiences.
• Ability to work both independently and as part of a collaborative team.
• Commitment to Housing First, harm reduction, trauma-informed care, and person-centered services.

Responsibilities

• Build relationships with individuals experiencing homelessness through outreach and engagement.
• Identify and engage youth aged 18-24 for services through street outreach.
• Perform assessments such as the Transition Age Youth - Service Prioritization Decision Assistance Tool (TAY-SPDAT).
• Provide supportive services to youth and other clients, assisting them in accessing stable housing, benefits, education, and health services.
• Maintain a caseload of up to 25 clients and develop individualized service plans.
• Advocate for clients to access services, benefits, and housing opportunities.
• Coordinate care with external partners to ensure continuity of services and reduce barriers.
