Job summary

Intercept Health is seeking a dedicated Program Manager for the Young Adult Independent Living Program. You will engage with residents aged 17–21, guiding them toward independence while modeling positive values.

The role includes supervising staff and planning activities tailored to each youth's strengths and needs. You will oversee money management, education, employment readiness, and daily living skills, ensuring individualized living plans are followed.

Qualifications

• Bachelor's degree in any field with 5+ years of case management experience required.
• Master's or Doctorate in social work or related field with 4+ years of casework experience in child-placing agency preferred.
• CSWE-accredited MSW/PhD with 3+ years of casework experience required.

Responsibilities

• Engage with young adults aged 17–21 preparing for successful community transition.
• Serve as a positive role model reflecting values and behaviors.
• Help residents develop money management, employment, education, daily living, and career skills.
• Supervise life skills coaches and support staff, including evaluations, scheduling, and disciplinary issues.
• Plan activities aligned with each youth's individualized transitional living plan.
