Job summary

Intercept Health seeks a Program Manager for the Young Adult Independent Living Program in Hampton, VA. The role guides residents aged 17–21 toward successful community integration, with emphasis on money management, employment, education, and daily living skills.

You will supervise life skills coaches and support staff, conduct evaluations, and coordinate scheduling while tailoring individualized transition plans to each resident's strengths and goals.

Qualifications

• Bachelor's degree required with five years of case management experience for children and families.
• Master's/Doctorate in social work or related field with relevant experience is acceptable.
• Experience in child-placing agencies and supervision of staff is preferred.

Responsibilities

• Supervise life skills coaches and support staff, including evaluations.
• Schedule staff and address disciplinary issues.
• Engage with residents aged 17–21 to support transition planning.
• Develop individualized living plans and monitor progress with residents.
