Job summary

New Avenues for Youth seeks a compassionate Case Manager to join our Housing Case Management team in Portland, OR. You will build trusted relationships with youth, support them in goal setting, navigate housing resources, and coordinate with service providers to move toward independence.

Qualifications

Two years of experience with high-risk youth.
Bachelor’s Degree in social work, psychology or related human services field a plus.
Experience with sexual minority and culturally diverse staff and youth.
Access to an automobile, valid driver’s license, and auto insurance.
Willingness to obtain CPR training and certification.

Responsibilities

Mentor and guide youth, building connections and helping them set and reach goals.
Advocate to navigate systems, coordinate with providers, and support housing independence.
Support growth through case planning and skill development.
Document progress with accurate records to inform program outcomes.
