Job summary

Qualifications

• Bachelor’s degree in a field related to the role is required.
• Community Health Worker certification required or ability to obtain certification within the program timeframe.
• 2+ years’ experience leading health-related programs, grants, contracts, and/or projects is required; youth program experience preferred.
• Ability to engage youth and families in a respectful, culturally responsive, and strengths-based manner.

Responsibilities

• Serve as a community-facing connection between youth, families, Corner Health Center services, health care providers, schools, and community resources.
• Conduct outreach, education, follow-up, and engagement activities for priority populations in schools and rural communities.
• Support screening, routine assessment, prevention, and risk-factor tracking activities.
• Identify barriers to care and help youth and families navigate health, behavioral health, social-service, school, and community resources.
• Facilitate referrals and warm connections to primary care, behavioral health, dental care, prenatal care, and other services; provide follow-ups.
