Job summary

MyMichigan Health is seeking a Community Health Worker to serve as a frontline liaison in Gratiot County. This temporary role requires conducting outreach and care coordination activities to connect patients with vital healthcare and community resources, all while addressing social determinants of health barriers.

The ideal candidate will work closely with community partners, promote health education, and maintain knowledge of best practices to improve health outcomes in the community.

Qualifications

• Experience in a health system, public health sector, or community benefit organization.
• Must have strong awareness of social determinants of health.
• Demonstrated ability to handle complex social/health issues.

Responsibilities

• Collaborate with school personnel to identify students in need.
• Manage patient outreach to address social determinants of health.
• Coordinate with the Community Health team for outreach events.
• Maintain knowledge of community resources and referral processes.

Skills

Strong written and oral communication skills

Knowledge of SDOH best practices
