Job summary

Corner Health Center in Sturgis, MI is seeking a Community Health Worker (CHW) Health Educator to connect youth, families, schools, and health services. • Bachelor’s degree in a field related to the requirements of the role is required.

• 2 or more years’ experience leading health-related programs, grants, contracts, and/or projects is required; experience working with youth and leading programs is strongly preferred.
• Ability to work independently and collaboratively across clinical, program, school, and community settings.
• Ability to engage young people and families in a respectful, judgment-free, culturally responsive, and strengths-based manner.

Responsibilities

• Serve as the primary CHW for Sturgis Student Help and Healing Center and connect youth, families, schools, and community resources.
• Conduct outreach, education, follow-up, and engagement activities for priority populations in schools and rural communities.
• Support screening, routine assessment, prevention, and risk-factor tracking activities related to chronic disease and other health priorities.
• Identify barriers to care and help navigate health, behavioral health, social-service, school, and community resources.
• Facilitate referrals and warm connections to primary care, behavioral health, dental, prenatal, preventive services, and other resources; provide follow-ups.
• Help identify gaps in chronic disease tracking and communicate findings to program and clinical leadership.

Job SummaryThe Community Health Worker (CHW) Health Educator is responsible for connecting young people, families, schools, community partners, and health care services to improve access to prevention, early intervention, education, and coordinated care. The position includes health education and positive youth development responsibilities and serves as the primary Community Health Worker (CHW) for Sturgis Student Help and Healing Center. The role conducts outreach and education; supports screenings, routine assessments, and risk-factor tracking; assists with preventive-care scheduling and follow-up; facilitates referrals and connections to health, behavioral health and social services; supports care coordination; collects and reports program data; and strengthens partnerships across health, school, and community systems.

Essential Duties and Responsibilities

Community Health Worker & Care Coordination

• Serve as a community-facing point of connection between youth, families, Corner Health Center services, health care providers, schools, and community-based resources.
• Conduct outreach, education, follow-up, and engagement activities for priority populations, including children and adolescents, in schools, and throughout rural communities
• Support screening, routine assessment, prevention, and risk-factor tracking activities related to chronic disease and other identified health priorities.
• Identify barriers to care and help youth and families navigate health, behavioral health, social-service, school, and community resources.
• Facilitate referrals and warm connections to primary care, behavioral health, dental care, prenatal care, preventive services, social services, and other appropriate resources; provide follow-ups
• Help identify gaps in chronic disease tracking, preventive care, services, community resources and communicate findings to program and clinical leadership.

Health Education & Youth Development

• Implement health education programs and services, including administrative, operational, and reporting responsibilities.
• Facilitate group-based health education activities and programs at Corner Student Help and Healing Center, in schools, and in community settings, including participant recruitment and outreach.
• Co-develop, adapt, and deliver age-appropriate health education curricula using evidence, best practices, health education standards, and priorities identified by young people.
• Facilitate discussions, trainings, workshops and educational activities that promote healthy decision-making, prevention, leadership, self-advocacy, connection to resources and health and wellness initiatives

Data, Evaluation & Quality Improvement

• Establish and maintain accurate and timely reports, records of outreach, education, screenings, referrals, follow-up, appointments, partnerships, and individuals served.
• Support collection of baseline information, participate in needs assessments for children and adolescents, support evaluation and measurement activities, all related to care coordination, partnerships, chronic disease monitoring, and prevention program.
• Perform other duties as assigned.

Working Conditions/Special Conditions

This position works across clinical, community, school, and other public-facing environments and may require regular local travel and evening or weekend activities based on program and community needs. The position may operate in a patient-care environment with exposure to body fluids, infectious disease, and potentially hazardous chemicals. Employees are expected to comply with Corner Health Center safety, infection-control, confidentiality, and workplace policies.

Position Schedule
