Qualifications

• Minimum associate degree; bachelor’s degree in human services or social work preferred.
• Strong ability to connect with culturally diverse children and families.
• Organized self-starter with ability to set priorities and meet deadlines.
• Excellent written, oral and interpersonal communication skills.
• Internet and Excel literate; valid driver’s license with reliable transportation.

Responsibilities

• Direct Patient Care: conduct screenings for emotional, behavioral, and developmental needs.
• Act as liaison between youth, families and Telebehavioral Health clinician.
• Provide education and support to promote healthy behavior change.
• Help families identify service needs and link to community resources.
• Proactively outreach to engage students and families for continuity of care.
• Care Coordination: collaborate with BH Clinicians to address needs and goals, participate in team meetings, document in EHR.
• Navigate internal/external medical, behavioral, social, service, and education systems; build community networks.
