Job summary

Bluebonnet Trails Community Services seeks a Wraparound Facilitator to deliver high-fidelity, family-driven Wraparound facilitation and care coordination under the Clinical Program Manager. You will partner with youth, families, schools, and providers to develop and monitor Wraparound Plans of care and coordinate services to achieve goals.

This role requires a Bachelor's in Human Services, a strong understanding of mental health, and a Texas driver’s license.

In support of our mission to continuously improve the health and independence of the individuals we serve, the Wraparound Facilitator provides high-fidelity, family-driven, youth-guided Wraparound facilitation and care coordination services under the supervision of the Clinical Program Manager. The Wraparound Facilitator partners with youth, families, natural supports, providers, schools, and community agencies to identify strengths and needs, facilitate Child and Family Team meetings, develop and monitor individualized Wraparound Plans of care, coordinate services, and support families in achieving their goals. The Wraparound Facilitator promotes collaboration, community integration, crisis prevention and planning, and the development of sustainable natural supports while ensuring services are delivered in accordance with YES Waiver, agency, contractual and state requirements. This position is responsible for coordinating services across multiple systems, monitoring progress toward identified outcomes, advocating for youth and families, and ensuring continuity of care throughout enrollment in the YES Waiver program. The Wraparound Facilitator maintains timely and accurate documentation in the electronic health record, manages multiple priorities and deadlines, and complies with all productivity, quality, and documentation standards. A minimum of 55% of working hours must be dedicated to direct service delivery. Because services are family-driven and youth-guided, work schedules may include evening hours, as needed, to accommodate the needs and availability of youth and families. Successful Wraparound Facilitators are strengths-based, culturally responsive, and committed to empowering youth with serious emotional disturbances and their families to remain safely and successfully in their homes, schools, and communities. They demonstrate excellent facilitation, communication, care coordination, advocacy, crisis response, and problem‑solving skills while maintaining professionalism, collaborating effectively with multidisciplinary teams, and actively participating in supervision, training, and continuous quality improvement initiatives.

Differential Detail

• $1.00 per hour – 2 years previous experience
• Up to $4.00 per hour – BTCS Tenure
• $0.75 per hour – Bilingual
• $1.00 per hour – Advanced Licensure

The Wraparound Facilitator must hold a Bachelor’s degree in Human Services*.

Possess a strong understanding of mental health diagnoses and treatment.

The ideal candidate demonstrates the ability to work both independently and collaboratively in a fast-paced, recovery-oriented environment, with a proven ability to build meaningful connections with individuals served.

A valid Texas driver’s license is required, along with the ability to maintain approved driver status throughout employment.

• Candidates whose degree is not in Human Services must have completed a minimum of 24 credit hours in a human services-related field.

Preferred Qualifications

A Master’s degree in Human Services or a related field is preferred

At least one year of experience providing wellness and recovery support services in mental health, substance use treatment, and/or crisis intervention and screening.

Candidates who are actively pursuing clinical licensure in counseling, behavior analysis, social work, family therapy, or a related discipline are encouraged, with preference given to those holding advanced or full licensure.

Bilingual proficiency in Spanish is highly valued.

Experience with concurrent (real-time) documentation practices is desirable.

Current licensure as a Licensed Chemical Dependency Counselor (LCDC).

Hourly Salary Range: $22.22 – $28.97
