Job summary

Bluebonnet Trails Community Services in Cedar Park, TX is seeking a Case Manager to provide recovery-oriented services under the supervision of the Clinical Program Manager. You will conduct assessments, develop Person-Centered Recovery Plans, and connect clients with community resources.

The role emphasizes direct service (minimum 55%), strong documentation in the electronic health record, crisis intervention when needed, and collaboration with a multidisciplinary team to support consistent

Qualifications

The Case Manager must hold a Bachelor’s degree in Human Services.
Strong understanding of mental health diagnoses and treatment.
Ability to work independently and collaboratively in a fast-paced recovery-focused environment.
Must have a valid Texas driver's license and maintain approved status.

Responsibilities

Conduct routine psychosocial and emotional needs assessments.
Develop Person-Centered Recovery Plans.
Connect youth, adults and families with community resources.
Provide rehabilitative services and initiate crisis intervention when necessary.
Document services in the electronic health record and ensure compliance.
