Job summary

Qualifications

• Minimum Bachelor's degree in human services or related field.
• Minimum 1 year of experience working with at-risk youth & families and case management.
• Flexible schedule with variable hours including day, evening, and on-call hours.
• Subject to call: Family Specialist on-call rotates with Program Coordinator; may require evening/weekend field work.
• Valid Driver's License with acceptable driving history; travel is required.

Responsibilities

• Assess family's strengths and needs; utilize treatment modalities and resources to effect growth and change.
• Develop, implement and evaluate comprehensive treatment and discharge plan; arrange alternative placement or permanency resources for youth if needed; complete discharge summary eight weeks prior to discharge.
• Deliver case management services; ensure counseling sessions are scheduled; ensure required medical and dental examinations; track services and maintain timely documentation with confidentiality.
• Conduct at least bi-monthly home and community visits to focus on treatment goals; build caring relationships with youth and families; ensure safety and secure environments.
• Develop and maintain relationships with community schools and resource agencies to support educational experience; coordinate with courts, probation, and local law enforcement.
