Qualifications

• Bachelor's degree in human services or related field.
• Minimum 1 year of experience working with at-risk youth & families and case management.
• Flexible schedule with variable hours including evenings and on-call rotation typically once per month.
• Subject to call: Family Specialist is required to be on call in collaboration with a Program Coordinator.
• Must possess Valid Driver's License and be able to travel up to 50% within Madison County.

Responsibilities

• Assess family's strengths and needs; utilize treatment modalities to effect growth and change.
• Develop and evaluate comprehensive treatment and discharge plans; create alternative placement resources.
• Deliver case management with timely documentation and confidentiality compliance.
• Conduct casework contacts with families in homes and community settings to meet goals.
• Develop and maintain relationships with schools and community resource agencies.
