connections, and available formal and natural supports. Communicates with the Child and Family Team
(CFT) including accessing the family's IP-CANS ratings to incorporate into planning and to provide ongoing
updates as well as receive vital information from the CFT.
  2. ENGAGEMENT: Leads the CFT in developing and implementing an individualized Plan of Care. Ensures
that the plan includes clearly defined goals, strategies, assigned responsibilities, timelines, and expected
outcomes. Promotes the meaningful participation of the child/youth and family/caregiver in all phases of
planning and decision-making. Helps them develop the knowledge, confidence, skills, and ownership
necessary to direct and eventually manage their own plan.
  3. FIELD BASED AND CULTURALLY RESPONSIVE SERVICES: Collaborates with schools, healthcare
providers, child welfare, probation, regional centers, community organizations, and other involved systems
to promote continuity of care and reduce fragmented or duplicative services. Identifies barriers to
engagement or progress and works with the CFT to develop practical, individualized solutions.
  4. PLAN OF CARE: Regularly monitors the implementation and effectiveness of the Plan of Care.
Reconvenes the CFT and modifies goals, strategies, services, or supports when circumstances change,
barriers arise, or the plan is not producing the intended outcomes. Assists the child/youth and
family/caregiver in identifying, developing, and strengthening natural and community-based supports that
can continue after formal services end.
  5. CRISIS RESPONSE: Collaborates with the child/youth, family/caregiver, CFT, clinical staff, and other
designated responders to develop and update individualized crisis and safety plans. Participates in crisis-response coordination within the scope of the position and follows Agency procedures when urgent safety or behavioral health concerns arise. Maintains awareness of client, family, staff, and environmental safety during all services and follows established treatment, crisis, safety, and agency procedures. Provides crisis
prevention, de-escalation, stabilization, and post-crisis follow-up within training and scope; promptly
communicates changes in risk to the appropriate clinician or supervisor and seeks immediate clinical or
emergency assistance when circumstances exceed the employee's scope or available resources. Follows
mandated reporting requirements and completes required incident and safety reporting within established
timeframes. During office hours, supports assessment and crisis management services, including coordination of an in-
person crisis response and facilitation of an involuntary psychiatric hold when determined to be clinically indicated by an appropriately qualified professional.
  6. AFTER-HOURS CRISIS ACCESS AND ON-CALL RESPONSIBILITIES: Supports the program's
responsibility to ensure that clients experiencing a crisis after regular business hours are connected to an
appropriate team member. Must serve as a direct connection to the FSP-HFW on-call team which may not
be replaced by directing clients exclusively to the LACDMH ACCESS Line, 911, or a suicide and crisis
    hotline.
  7. CO-OCCURRING SERVICES: Supports children and youth living with co-occurring mental health and substance use conditions through integrated, developmentally appropriate, trauma-informed, and family-centered care. Within the employee's qualifications and scope, identifies observable substance use concerns, overdose risks, and changes in functioning; communicates concerns to the appropriate clinician
or supervisor; and assists with referrals and coordination of clinically necessary services.
Supports the team in the following activities:
    a. Providing or linking clients to individual or group-based interventions that may include motivational
interviewing, 12-step approaches, Acceptance and Commitment Therapy, or other approved
approaches intended to increase understanding of problematic substance use behaviors.
    b. Applying harm-reduction principles throughout the recovery process to help children, youth, and
families reduce the risks and adverse effects associated with substance use and its impact on
    mental health.
    c. Providing referrals and linkages to community substance use disorder resources and supports,
including age-appropriate self-help groups, outpatient and residential programs, detoxification
    providers, and recovery-support services.
