Job summary

Integrated Services for Behavioral Health (ISBH) in Athens County, OH is seeking an Engagement Care Coordinator to assist in immediate care coordination needs for youth enrolled in OhioRISE. The role involves supporting families with discharge planning, educating about OhioRISE, and referrals for follow-up care in home, school, and community settings.

The position pays $20.19-$25.03 per hour and requires driving; successful candidates will participate in engaging with families, cross-systems

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health.
• Two years of experience in a coordinated supportive services or care coordination role preferred.
• Be culturally humble or responsive with training and experience to manage complex cases.

Responsibilities

• Joins with family to identify care coordination needs/services in line with standards and program outcomes.
• Works with families to define cultural factors influencing strengths and engagement in care planning.
• Identifies strengths of children, youth, and families for utilization in care coordination.
• Coordinates family-based-services for children, youth, and families in home, school, and community.
• Ensures services identified on care plans are most appropriate, least restrictive, and meet safety needs.
• Engages and builds positive relationships with families to support integration of team members and plans.
