Job summary

Integrated Services for Behavioral Health (ISBH) in Ohio seeks an Engagement Care Coordinator to support youth hospitalized under the OhioRISE program. You will assist families with discharge planning, educate about OhioRISE, and arrange referrals for follow-up care and services while attending meetings to complete engagement milestones.

The role requires strong organizational skills, cultural humility, and experience with multi-system care coordination.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health or related fields.
• Three years with a high school diploma or equivalent; two years with an associate or bachelor’s; one year with a master's degree or higher.
• Hi-Fidelity Wraparound knowledge preferred; certification provided at time of employment.

Responsibilities

• Identify care coordination needs with families to align with service standards and outcomes.
• Define cultural factors influencing strengths and family interaction for ongoing engagement.
• Coordinate family-based services in home, school, and community.
• Attend in-person and virtual meetings to complete engagement milestones and warm handoffs.
