Job summary

Integrated Services for Behavioral Health (ISBH) seeks an Engagement Care Coordinator in Muskingum County, OH. The role focuses on assisting immediate care coordination for youth enrolled in OhioRISE, supporting families through discharge, and linking them to follow-up services.

The position requires working with families, schools, and community partners to develop care plans, safety plans, and team meetings, ensuring culturally aware, child-centered care.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health or related public sector services.
• Knowledge of wraparound services and impact on youth outcomes is preferred.
• Ability to work with diverse families and coordinate across systems.

Responsibilities

• Identify care coordination needs with families and teams to support children and youth.
• Coordinate family-based services in home, school, and community settings.
• Develop and implement care plans and safety plans with families and teams.
• Attend meetings (in-person/virtual) to progress engagement milestones.
• Maintain documentation and participate in program evaluation and cross-systems training.
