Job summary

Allendale is seeking a Case Manager to provide community-based service coordination, crisis intervention, and placement support for youth and families in need. You will work closely with court systems, schools, and community partners to facilitate reunification and stability.

Responsibilities include conducting intake and risk assessments, developing service plans, linking clients to resources, and maintaining documentation in HMIS and EHR systems.

Qualifications

• Bachelor’s degree required; human services or related field preferred.
• Excellent written and verbal communication skills.
• Valid driver’s license with clean driving record.
• First Aid and CPR certification within first 90 days.
• Working knowledge of word processing software (MS Word, etc.).

Responsibilities

• Conduct crisis/safety assessments on-site and in community settings.
• Assist youth and caregivers in identifying and utilizing community resources.
• Conduct intake assessments and comprehensive assessments using standard protocols.
• Manage case planning, resource linkage and service coordination with families and professionals.
• Maintain accurate records in the agency’s electronic health record and related systems; prepare reports for meetings and court proceedings.
