Job summary

Southwest Network, Inc. is seeking a Case Manager to facilitate the Child and Family Team process and develop individualized service plans for youths with behavioral or emotional needs. You will coordinate linkages to community resources and health care, while partnering with families and professionals.

The role emphasizes comprehensive care planning, collaboration, and ongoing support to help youths stay connected with home, school, and community resources.

Qualifications

• Must have a behavioral health or related field degree as listed.
• Must maintain CPR, First Aid, and Therapeutic Options certifications.
• Must have valid Arizona fingerprint clearance card.
• Must have valid Arizona driver's license and car insurance.
• Must be at least 21 years old.

Responsibilities

• Facilitate the Child and Family Team process to develop individualized service plans.
• Coordinate linkage to community resources and health care services.
• Collaborate with families and others involved in the youth's life.
• Ensure plans address behavioral and physical health needs.
