LE001 Community Services Group, Inc. is seeking a Case Manager to provide casework services for children and families within the Blended Case Management program. You will conduct assessments, develop goal-oriented service plans, coordinate needed services, and maintain accurate records to support family stability.

The role requires flexible hours, travel to client locations, and participation in on-call coverage. Starting wage is $20.50/hour, negotiable based on experience.

Qualificações

• Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, criminal justice, nursing, political science, history, counseling, or education.
• RN or HS diploma with 12 credit hours in sociology/social welfare/psychology/gerontology or related field and 2 years in human services with 1 year direct client contact.
• HS diploma with 5 years of mental illness direct care experience in human services with prior intensive case management.

Responsabilidades

• Interview individuals and representatives to gather demographic and psychosocial information.
• Develop strength-based assessments and need analysis with individuals and families.
• Develop goal-oriented, time-limited service plans with the individual/representatives.
• Coordinate needed services for individuals and families and others in the plan.
• Maintain accurate, complete records and produce clear reports.
• Participate in training on company purpose, services, laws, and procedures.
• Make referrals to public/private social services and resources.
• Maintain regular communication with families, schools, physicians and service providers.
• Travel to locations for home-based services and attend trainings/appointments.
• Perform on-call services and provide coverage in emergencies.
• Possess knowledge/experience in the child welfare system; conduct interviews to identify issues and goals.
• Make sound decisions and manage stressful situations.

Formação académica

Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, criminal justice, nursing, education, or related field

RN or HS diploma with social science coursework plus 2 years public/private human services with 1 year direct client contact OR HS diploma with 5 years mental illness direct care experience and prior intensive case management
