Job summary

Community Services Group - CSG is seeking a Case Manager to provide comprehensive casework services for children and families. You will use clinical and casework skills with significant independent judgment to support personal growth and social stability.

Responsibilities include assessments, service planning, referrals, and coordination with families, schools, and providers. Requires flexible hours, driving, and knowledge of child welfare. Starting wage is $20.50/hour.

Qualifications

• A bachelor’s degree from an accredited college or university with major coursework in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, political science, history, counseling, or education; OR Registered nurse or High school diploma or equivalency and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and 2 years experience in public or private human services with 1 year in direct client contact or High school diploma or equivalency and 5 years of mental illness direct care experience in public or private human services with employment as an intensive case management staff person prior to April 1, 1989.

Responsibilities

• Interview individuals and representatives to gather demographic and psychosocial information.
• Complete strength-based assessments and need analyses with individual and family.
• Develop goal-oriented, time-limited, comprehensive service plans with the individual and/or representatives.
• Coordinate needed services for individuals, families and others who are significant to the service plan.
• Maintain accurate, complete individual records and produce clear, accurate reports.
• Participate in formal and informal training programs addressing the company’s purpose, services, population, and regulations.
• Make referrals to public/private social services and resources to meet needs and assist families in utilizing these resources.
• Maintain regular communication with families, schools, physicians and other service providers.
• Travel to locations, other agencies, and community resources to arrange and provide community and home-based services for children, adolescents, and families.
• Perform on-call services and provide coverage in emergencies.
• Knowledge of or experience in the child welfare system.
• Ability to conduct interviews and use them to identify issues and goals.
• Ability to make sound decisions and handle stressful situations.
