Integrated Services for Behavioral Health (ISBH) is seeking an OhioRISE Care Coordinator to lead a team supporting youth and families in home, school, and community settings. The role focuses on identifying needs, goals, and action steps with families, partners, and service providers to improve outcomes and minimize hospitalizations.

Qualifications

• Experience providing services and/or support to children and families in behavioral health or related public sector human services.
• Hi-Fidelity Wraparound knowledge is preferred (Certification provided after employment)
• Two years of experience in a care coordination or coordinated supportive services role
• Experience with autism spectrum disorders and developmental disabilities preferred
• Knowledge of family systems, community resources, case management, and child development

Responsabilités

• Lead and organize care coordination for youth and families at home, school, and community.
• Develop, lead, and facilitate team meetings with the young person and identified team.
• Maintain documentation to access and sustain OhioRISE benefits.
• Ensure confidentiality and HIPAA compliance in all activities.
• Travel within assigned service area and participate in trainings and supervision.
• Collaborate with service providers and family supports to achieve goals.
• Utilize EHR systems for accurate client documentation and reporting.
• Meet program fidelity and quality requirements.
• Complete all assigned trainings within timelines.
