Job summary

Integrated Services for Behavioral Health (ISBH) in Ohio is seeking a Care Coordinator to support youth and families through the OhioRISE program. You will help plan, organize, and lead a multidisciplinary team that works with families at home, in school, and in the community to keep families intact and reduce hospitalizations.

The role requires coordinating care, ensuring youth and family voices are heard, and traveling to visit clients while collaborating with social service partners.

Qualifications

• Experience providing services to children and families in behavioral health.
• Two years in a coordinated care role with high efficiency and organization.
• Experience with autism spectrum disorders and related supports.
• Ability to bill Medicaid without restrictions and drive to visits.

Responsibilities

• Lead child and family team meetings to plan care.
• 1 Develop, lead and facilitate team meetings for the youth and their identified team.
• Provide care coordination in the home, school, or community.
• Collaborate with families, natural supports, and service providers to meet needs.
