Job summary

Family & Children’s Center in La Crosse, WI seeks a Case Manager for the Coordinated Specialty Care program helping youths aged 15-25 access services and develop recovery skills within the community.

The role requires a master’s level education, CBT knowledge, family support, and ability to travel to client homes, collaborating with a multidisciplinary team to ensure timely care and documentation.

Qualifications

• Education, training and work experience aligned to a master’s level degree with supervised clinical hours.
• Knowledge in CBT, family education and support, and substance use disorders.
• Ability to use common office equipment and enterprise software; valid driver's license and own vehicle for client transport.

Responsibilities

• Develop CSC program processes in line with grant requirements.
• Coordinate with external providers for resources and referrals.
• Provide assessment, treatment planning and symptom management.
• Engage families and support systems.
• Attend team and client-related treatment meetings.
• Coordinate client care across psychotherapy, skills training, housing and counseling.
• Ensure timely completion of client service paperwork.
