Job summary

Delaware Guidance Services for Children and Youth, Inc. seeks an Engagement Specialist to coordinate care for children and youth in crisis. Under supervision, you will conduct outreach, reconnect former clients, and deliver caring contacts, ensuring continuity of care and timely referrals.

You will collaborate with internal teams and community partners to address ongoing needs, navigate systems, and advocate for youth and families in a culturally competent, client-centered manner.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
• Two years of experience in case management, healthcare, mental health services, or crisis intervention programs.
• Familiarity with Delaware’s behavioral health systems, state regulations, and community resources for children and families.
• Ability to communicate effectively with diverse populations and families.

Responsibilities

• Implement Caring Contacts protocol through calls, mailers, and electronic communication to reduce suicide risk.
• Conduct outreach to discharged clients to encourage participation and support.
• Maintain contact with former clients and families to assess ongoing needs and referrals.
• Coordinate services and follow-ups by phone and email, ensuring timely information exchange.
• Serve as liaison between crisis program, community agencies, schools, and other stakeholders to coordinate services.
