Job summary

St. Joseph's Villa in Virginia is seeking a Clinical Case Manager to coordinate crisis services for children ages 5–17, including CRC and CSU.

You will lead intensive case coordination using wraparound, link families to community resources, and partner with CSBs, schools, physicians, and providers to ensure warm hand-offs and seamless discharge planning. The role requires a BS/BA in Psychology or related field (Master's preferred), two years of related experience, proficiency with Windows and MS

Qualifications

• BS/BA in Psychology, Human Services, or related field; master's degree preferred.
• Minimum two years of related experience; QMHP preferred.
• Excellent verbal communication and ability to work with parents/guardians.
• Proficient with Windows, Outlook, and MS Office; familiarity with proprietary software.
• Valid Virginia driver’s license with excellent driving record.

Responsibilities

• Maintain up-to-date resource lists and develop partnerships for warm hand-offs.
• Provide intensive case coordination using High-Fidelity Wraparound model.
• Collaborate with Crisis Services to deliver treatment and direct care.
• Screen referrals and determine eligibility for services.
• Coordinate tele-psych psychiatry and ensure continuity of care.
• Develop action plans and crisis prevention plans with families.
• Discharge planning and linkage to resources post-discharge.
• Document case management and prepare discharge summaries.
• Attend staff meetings and trainings supporting Crisis Services.
• Pursue continuous quality improvement opportunities.
