Job summary

St Joseph’s Villa in Virginia is seeking a Clinical Case Manager for Crisis Services programs serving youth aged 5–17. Under the Clinical Supervisor, you will coordinate services across the CRC and CSU, linking to external agencies and ensuring discharge planning for ongoing stabilization.

Knowledge of Crisis Now Model, de-escalation techniques, Medicaid billing, and collaboration with families, schools, physicians, and CSBs is essential.

Qualifications

• BS/BA in Psychology, Human Services or related field; master’s preferred.
• Two years of related experience; QMHP preferred.

Responsibilities

• Maintain up-to-date resource lists and develop partnerships for crisis stabilization.
• Coordinate intensive case management with wraparound model and CSB collaboration.
• Link individuals and families to community resources with warm hand-offs.
• Document case management activities and prepare discharge summaries.
• Participate in trainings and team meetings as required.
