Knowledge of Crisis Now Model, de-escalation techniques, Medicaid billing, and collaboration with families, schools, physicians, and CSBs is essential.

Qualifications

  • BS/BA in Psychology, Human Services or related field; master’s preferred.
  • Two years of related experience; QMHP preferred.

Responsibilities

  • Maintain up-to-date resource lists and develop partnerships for crisis stabilization.
  • Coordinate intensive case management with wraparound model and CSB collaboration.

Overview

Under the direction and supervision of theClinical Supervisorfor Crisis Services,the Clinical Case Managerworks with children ages 5 – 17 in Crisis Services programs at St Joseph’s Villa. Crisis Services includes both the Crisis Receiving Center (CRC) and the Crisis Stabilization Unit (CSU). Clinical Case Managercoordinates the overall services provided toindividuals with external agencies andprovides targeted discharge linkages forindividuals to ensure ongoing stabilization for the nextappropriate levelof care.

All aspects of service delivery are to be conducted in alignment with the Crisis Now Model**– No Wrong Door/No Force First approachand systems-based framework with strong collaboration skills with and in support of other services offered by St. Joseph's Villa and the community (partnership with Region 4),All work is performedin accordance with the highest professional ethical standards, and in alignment with Saint Joseph’s Villa mission, values, and guiding principles.

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of the philosophy and practices of therapeutic programs for the emotionally, behaviorally, and cognitively impaired individual. Knowledge ofappropriate methodsof de-escalation and ability to implement those methods appropriately. Knowledge of Medicaid Billing procedures. Able to work effectively with parents or guardians and placing agencies. Excellent verbal communication skills. Proficient in basic mathematics and elementary statistical techniques. Able to organize case load to maximize timeutilizationwith no diminution in quality of service. Able to appropriately use proprietary software. Able to work effectively ina collaborative, shared leadership environment. Able to be flexible and appropriately handle stressful situations.

Work Schedule: Monday - Friday: 9:00am - 5:00pm with rotating weekends (1 weekend every 3 weeks)

Salary: $65,000

Work Status: Full-Time Exempt

Responsibilities

ESSENTIAL JOB FUNCTIONS

Responsible formaintainingup to date resource lists and developing partnerships with external parties to ensure effective linkage with a warm hand-off to support crisis stabilization.

Provides intensive case coordination using theHigh-FidelityWraparound model forindividuals and families in coordination with Community Service Boards(CSB)and otherappropriate organizationincluding but not limited toschools, physicians, private providers, etc.to support stabilization upon discharge.

Works collaboratively with theCrisis Servicesteamto providethe highestlevel of treatment services to individuals and families andprovidenecessary direct careforindividuals.

May conduct referral screeningsand consult with crisis services teamtodetermineeligibility for services.

May actas a liaison between tele psych psychiatrist and crisis stabilization teamin order toensure continuity of care.

Works withtreatmentteam to develop action plans and crisis prevention plans based on the family’s prioritized needs.

Workscollaboratively withcrisis servicesteambased on the Individual’s and family’s prioritized needstoidentifyeffective treatment, develop safety plans,and discharge planningfor continuous stabilization upon discharge.

Researchescommunity-based organizations and governmental agencies todeterminewhat resources are available to support theindividual’s needs.Actively linksindividuals and their families to these identified resources.

Maintains case managementdocumentation.

Compiles information to prepare discharge summary reports ofindividuals.

Maintains contact with designated family members or guardians and school systems.

Coordinates community support and intra-agency support as needed foreach individualto ensure effective linkage with a warm hand-off to supportcrisesstabilization.

Modelshigh levelof professionalism withappropriate communicationwith individuals and staff, behavior, and dress.

Attends andparticipatesin All-Staff Meetings, weekly Treatment Team Meetings,training, and other events/meetings that support the operation of Crisis Services

Evaluates work processes and methods andsubmitsrecommendations for continuous quality improvement.

Attends scheduled training programs for professional development that includes, at a minimum, trainings required by St. Joseph’s Villa and by regulatory and accrediting bodies.

ADDITIONAL DUTIES AS ASSIGNED BY SUPERVISOR

Mayparticipatein individual intake, admission, and discharge process

May coordinate andparticipatein tele-health psychiatry services

May administer medication.

May provide directindividualcareas needed.

When necessary, may be deployed to other services at St Joseph’s Villa to provideappropriate neededservices.

Performs other duties as assigned.

Qualifications

QUALIFICATIONS

BS/BA degree in Psychology, Human Services, or related field.A master’sdegree is preferred.Twoyears ofrelated experience. One year of clinical experience with children (QMHP preferred). Proficient with Windows, Outlook, MS Office including Word,ExcelandPowerPoint.Experience with proprietary software preferred.Valid Virginia driver’s license with excellent driving record
